Release Forms and SRC Forms
Two types of forms are required for the State Science & Engineering Fair:
1Release forms are required of all Fair attendees, students and adults. These forms are: Parental Consent, Emergency Medical Information, Code of Ethics, a Media Release Form, and a Waiver Form. You must print out Release Forms during the registration process and mail signed forms to the MAS office at the address listed below. New for 2013: The office is no longer accepting emailed or faxed Release Forms. If you need additional blank Release Forms, use these Links:

All students who win cash awards will need to fill out a W9 Form after the event to collect their cash.

2SRC Forms are required of all student presenters. SRC forms are used to verify that students' research is done following appropriate scientific methods. Blank SRC forms come directly from the international competition (ISEF) or its website. Each student fills out different forms depending on their research. You should have already filled out the SRC forms to compete at your Regional Fair.
STUDENT PRESENTERS - New for 2013: All student SRC forms that were approved and signed off at Regions are to be given to your Regional Director for mailing to our office. Make sure to keep your original SRC paperwork, give the "working copy packet" of your SRC forms to your Regional Director. Directors will mail student SRC packets to the MN Academy of Science office - State Science & Engineering Fair, 8700 West 36th Street, Suite #114W , St Louis Park, MN 55426.
If corrections need to be made, the State SRC will send the "working copy" back to regional director. Please make certain you are using the SAME WORKING COPY that you gave to regional fair director to make the corrections before you send back to State.
While every student will have different SRC forms to submit, everyone must have at least these SRC Forms include:
- Checklist for Adult Sponsor (1)
- Student Checklist (1A)
- Approval Form (1B)
- Research Plan (Must include: Problem, Hypothesis, Procedure, Bibliography with 5 references - website references must include date viewed)
- Abstract
Additional forms are required based on your specific project. Common additional forms include: 1C (work done at an institutional or industrial setting), 4 (human subjects), 5A (working with Vertebrate animals), 6A (working with potentially hazardous biological agents), (7 (continuation of earlier project). Make sure to keep your original SRC paperwork, and bring a copy with you to the State Fair. Your Regional Fair organizers will be able to help you with the appropriate ISEF forms needed. For more For more information, check the ISEF site.
Avoid common SRC Violations:
- Don't forget to send us your Bibliography
- Don't forget to send us your Research Plan
- Be careful about all dates on Form 1A, Form 1B, and Form 1C - Some dates MUST be before or after the start / end dates of your project, read the directions carefully by each date field.
